Frequently asked questions
What should I write in a symptom diary?
Write the date, the symptoms you noticed and a 0–10 rating for each one. Add sleep hours, food, medications and any brief notes you want to remember. Use the same rating scale each day.
How do I print the diary or save it as a PDF?
Select Print / Save as PDF to open your browser print dialog. Choose Letter or A4 paper, portrait orientation and default scale. Choose Save as PDF instead of a printer to keep a digital copy. Turn off browser headers and footers for a cleaner sheet.
How do I share it with my doctor?
Bring the completed sheet to your appointment or ask your clinic how to send a PDF securely. Highlight the dates and notes you want to discuss, and keep a copy for yourself. Share only the details you feel comfortable sharing.
Do I need an account to use the printable diary?
No account is needed to view, print or save this blank sheet. If you prefer a digital daily log, you can start free in The Good Tracker. The app requires a free account and syncs your logs to your account.
What if I miss a day?
Leave that day blank or add a note that you did not record it. Continue with the next date. A blank entry is different from a symptom score of zero, so only write zero when you mean none.
